Eligibility: Entries will only be accepted from individuals aged 18 years and over who are employed by NSW Government local health districts, on a permanent full time, part time or casual basis at the following agencies South Western Sydney LHD, Mid North Coast LHD, NSW Ambulance, Western NSW LHD, Murrumbidgee LHD at the time their entry is drawn.
How to enter: Eligible NSW Government local health districts employees whosign up for any of the available benefits (salary packaging, Meal Entertainment or a novated lease if eligible) during the competition period will receive one entry per benefit into the draw win a Park Hyatt Voucher to the value of $1,500 and a Flight Centre Voucher to the value of $500. Salary packaging and Meal Entertainment must be active by 12 October 2018. New novated lease customers must have vehicle ordered by 12 October 2018. Visit https://www.maxxia.com.au/nswlhd to sign up.
Existing customers: NSW Government local health districts employees who are already salary packaging all available benefits (salary packaging, meal entertainment and a novated lease where eligible), during the competition period and active as at 12 October 2018 will automatically go into the draw once.
Competition period: Competition opens 8:30 am 01 September 2018 and closes 28 September 2018 03:00 pm (AEST).
Prize details: The first eligible entry randomly drawn will win a Park Hyatt Voucher to the value of $1,500 and a Flight Centre Voucher to the value of $500 this includes;
Prize valid for 3 years from the date of issue. The prize, may not be transferred, exchanged or redeemed for cash or other goods or services.
Determining the winner: The random prize draw will take place at 3:00 pm (AEST) on 17 October 2018 at Maxxia’s offices at Level 21/360 Elizabeth Street, Melbourne VIC 3000. The first eligible entry randomly drawn, as determined by Maxxia, will be the winner.
Announcing the winner: Maxxia will announce the winner online on 23 October 2018 and will notify them by email and telephone call using the details provided with the winning entry. The prize winner will be published at https://www.maxxia.com.au/nswlhd.
Claiming the prize: The winner must claim their prize by 5:00 PM (AEST) on 17 January 2019 by confirming the receipt of the notification by return email, or during the notification telephone call. If the prize is not claimed by 5:00 PM (AEST) on 17 January 2019, the winner will be taken to have forfeited the prize, and Maxxia will randomly draw another winner from valid entries at 9:00 am (AEST) on 21 January 2019 at Maxxia's offices at Level 21/360 Elizabeth Street, Melbourne VIC 3000.
Maxxia will announce the redraw winner online on 23 January 2019 and will notify them by email and telephone call. The redraw winner will be published at https://www.maxxia.com.au/nswlhd.
Accepting any prize may attract Fringe Benefits Tax (FBT). This means that acceptance of this prize may impact a winner personally in terms of the fringe benefit being reported as a reportable fringe benefit in the winner's 2018/2019 payment summary. Before accepting a prize the winner may want to seek advice from their financial/tax advisor on the consequences of a fringe benefit being reported in the payment summary, and the impact this will have on certain Government benefits, levies and charges.
